Moving a player (typically down a division) can be an emotional and/or stressful experience for all parties (Player, Parents, Coaches) if not managed and communicated timely and appropriately. This form is primarily about process - not the Coaches decision to make a move.Coach (Your Name)* Your Email Address* Your Phone Number* Your Team - Gender, Age, Division* (E.g. Girls U12 Div2)Name and Number of Player to Move* What division is the Player moving to?* List the reasons for the Player Movement*List the actions taken to keep/correct the Player*Have you discussed the move with the JDO?* Yes No It is a requirement that all moves be reviewed and approved by the JDO.Have you advised the Parent AND Player of the move?* Yes No It is a requirements that the Coach advise the parent and player together to explain the approved move well in advance of the move date. Face-to-Face consultation is preferred at a pre-arranged meeting time.
